Baccalaureate degree in health services administration, nursing, public health or related field required. Master’s degree in related field required. Certified RCA facilitator preferred. Strong verbal/written and presentation skills required. Six Sigma/Lean Green Belt or Black Belt certification desired upon entry to position, certification must be completed within 18 months of accepting position.
Five years of relevant experience in patient safety programming, risk management, conducting FMEA’s and use of root cause analysis tools, facilitation of small and large groups/project teams, performance improvement systems and/or process improvement methods and standards. Project management experience, preferably in a large complex healthcare organization is desired. Knowledge of electronic event reporting systems (e.g., SafetyNet) and TJC patient safety standards is required.
The Director of Patient Safety reports to the Administrative Director of Operational Excellence. Under general supervision and within Lifespan established policies and procedures, directs and manages coordination, collection and analysis of patient safety data for internal comparison and external reporting through Lifespan’s electronic event reporting system (SafetyNet) to the Patient Safety Organization (PSO). Provides oversight for the medical event reporting workflow to ensure appropriate, timely root cause identification and analyses, and related improvement project timelines. Establishes and maintains processes to ensure timely event review to ensure relevant internal action as well as timely event data submission to the Patient Safety Organization via SafetyNet. Demonstrates technical knowledge of the SafetyNet system for the management and reporting of all patient safety events. Monitors for and facilitates implementation of changes to ensure effective quality assessment/performance improvement (QAPI) monitoring requirements of Lifespan and affiliates’ safety programs, The Joint Commission (TJC), Centers for Medicare and Medicaid Services (CMS), RI Department of Health and other regulatory agency requirements are met. Acts as liaison to Risk Management and oversees tracking and reporting systems ensuring the effectiveness of corrective actions identified through root cause analysis and improvement opportunities. Notifies the Administrative Director when issues arise and takes corrective action as needed.
