Bachelor’s degree in social services, psychology, public health or related field.

Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.


EXPERIENCE:

Written and verbal communication skills to interact effectively with medical professionals, schools, and community organizations.

Demonstrated knowledge of spreadsheet and office software applications.

Leadership and organizational skills to effectively recruit participants, mentors, residents, and volunteers, coordinate TALC activities, and prepare grant applications for continued funding.

Experience working with teens, families or children preferred.

Familiarity with grant application preparation preferred.


Under general direction of the clinical and administrative supervisors of The Adolescent Leadership Council (TALC), coordinates and facilitates all aspects of the TALC program. TALC promotes the successful medical transition of patients who live with chronic illness. Teens, their families and college student mentors, are educated about the transition process through one of the three tiers of TALC programming. TALC also provides training opportunities for medical students and residents. Responsibilities include coordinating and implementing all TALC programs, supervising all TALC volunteers, identifying and recruiting participants, training residents and volunteers, preparing funding applications and maintaining related records.
