High School diploma or equivalent. Successful completion of formal education in hospital-specific coding. Coding certification from the American Health Information Management Association preferred. Familiarity and understanding of the content of the medical record. Trained in anatomy, physiology and disease processes. Ability to recognize and understand clinical documentation pertinent for coding. Good writing skills to prepare physician queries. Computer skills to research internet websites to clarify diseases or procedures. Ability to access and recognize appropriate electronic documents for coding.


3-5 years inpatient coding experience reading medical records in an acute care facility.

Under the general supervision of the Manager, Coding and Data Quality, reads the medical record assigning the appropriate diagnostic and procedure codes (ICD-9) for claims processing, data retrieval and analysis. Knowledge of the Medicare Prospective Payment System relative to DRG assignment, accurately reporting and sequencing principal and secondary diagnoses and procedures. Queries physicians when documentation in the record is inadequate, ambiguous or unclear for coding purposes. Maintains quality and productivity standards coding records within 2-3 days of discharge.
