Case Worker Outpatient

Summary:

C&F Outpatient: Under the supervision of the Clinical Director of Child & Family Outpatient Services (CAFOS) of Gateway Healthcare and abiding by all applicable policies and procedures provides case management to clients and families in active treatment with Gateway CAFOS clinicians provides triage and support for clients in crisis supports outpatient treatment adherence assists in meeting screening requirements for clients and monitors required documentation timelines.

FRIENDS WAY: Under the direction of the FRIENDS WAY Program Director plans and coordinates the peer support group program in the urban communities of Central Falls Pawtucket and Providence.

 

Responsibilities:

Child & Family Outpatient Case Worker responsibilities include:

Participates in program related meetings including but not limited to staff meetings clinical conferences community meetings and in-services.

Completes documentation within Lifechart according to established policies and procedures.

Attends individual supervision with the Clinical Director of the Child & Family Outpatient Service.

Supports the collection and reporting of data required for measuring quality and meeting licensing requirements.

Provides families with resources such as outpatient therapy groups specific supports and higher levels of care as needed.

Answers calls from parents/guardians/clients that are in crisis when their provider is unavailable.

Maintains an active list of referral options in the community for additional services and helps identify services for clients who may be graduating or leaving the area.

Provides support and referral information for family members of active clients (e.g. parents siblings).

Runs Epic reports to identify clients who have not been seen in over 1 month for therapists and 3 months for prescribers and/or have frequent no shows throughout the year to support clients to follow their treatment plans.

Offers reminder calls to families who require extra support getting to appointments.

Supports Missed Appointment/No Show policy by reiterating the importance of regularly attending scheduled appointments providing logistic support when necessary setting limits with clients/families and sending out reminder letters and working with Risk Management to discharge clients according to the policy.

Coordinate care with community organizations to support identified families to overcome barriers to treatment such as homelessness rides to and from appointments and trouble obtaining medication.

Meets with clients 1:1 to complete screenings work on goal setting and how to achieve goals.

Collaborates with clinicians to support them and their clients.

Enters referrals and screening information in LifeChart for clients as directed by clinician.

Initiates medication prior authorizations on behalf of the providers.

Runs reports for clients with upcoming appointments that have outstanding lab work or screenings that needs to be completed and provides reminders to those families in advance.

FRIENDS WAY Case Worker responsibilities include:

Assists in developing outreach material in English and Spanish.

Recruits families to participate in the program particularly those from black and Hispanic/Latinx communities.

Assists in recruiting and training volunteer group facilitators from the community.

Works with the FRIENDS WAY Program Director to develop curriculum and activities for the weekly sessions.

Coordinates the weekly peer group sessions plan activities purchase and organize group materials coordinates space and food needs with Segue Institute arranges classroom spaces and facilitates sessions.

Develops activities and provides appropriate materials to accomplish the program goals for each age group.

Assists in conducting satisfaction surveys from families and facilitators.

     

    Other information:

    QUALIFICATIONS:

    A Bachelor�s degree in social work psychology or other health and human service degrees accompanied by at least one year of relevant experience in children�s behavioral health.

    Demonstrated knowledge and skills of human development stages and cultural patterns in each step of the care process.

    Bilingual in Spanish

    Knowledge of community resources specific to the cultural needs of the community

    EXPERIENCE: 

    Relevant case management experience required.

    Demonstrated ability to interact professionally both on the phone and with colleagues.
