Medical Records Clerk

Summary:
Provides general maintenance of both computer and paper clinical records exercising consistent and detailed attention to policies and procedures regarding access to client records confidentiality filing and data entry.

 

Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:

Sort documents for filing according to the clinical record number on a daily basis.



Files documents into the clinical record in an accurate and timely manner according to the agency Records Format and File Order on a daily basis.



Pulls records for scheduled appointments and re-files as necessary.



Assembles and repairs new records as needed.



Assists in obtaining records from other locations.



Maintains an adequate supply of clinical forms.



Scans and photocopies forms and other documentation as needed following procedures.



Conduct reviews of clinical records for completion of forms during the filing process on an ongoing basis.



Assists in the completion of various department projects.



Data entry of information into the CMHC clinical record screens.



Monitors the information to be entered for accuracy and completeness.



Notifies and submits reports to the appropriate Program Manager and supervisor.



Prints and distributes active caseload lists to clinical staff on a scheduled basis.



Answers the telephone as necessary.



Provides assistance or coverage at other agency locations.



Provides assistance with agency projects as requested.



Processes requests for information in accordance with HID procedures/policies.



Other duties and responsibilities as assigned or needed.

QUALIFICATION REQUIREMENTS:

High school diploma or general education degree (GED); a minimum of one year-related experience and/or training; or equivalent combination of education and experience.



NECESSARY SPECIAL REQUIREMENTS:

Valid drivers license.



JOB KNOWLEDGE SKILL AND ABILITY:

Ability to operate computer printer typewriter copy machine and fax machine



Ability to type accurately at a reasonable speed.



Familiarity with medical/psychiatric terminology.



Familiarity with policies and procedures.



Ability to remain calm in emergency situations.



Ability to perform duties independently and in a timely manner.



Ability to handle many tasks at the same time in an organized fashion.



Ability to read comprehend and interpret policy procedures instructions correspondence and memos.



Ability to write business correspondence.



Ability to effectively present information and respond to questions from managers clients and other staff members.



Ability to carry out instructions furnished in written oral or diagram form.



Ability to deal with problems involving several variables in common situations.



Ability to successfully complete all required training and certifications



Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.



Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals either manually or by use of a calculator.
