Nurse Practitioner PD

Provides health care services to individuals families and groups including health promotion disease prevention and health problems management. Provides outpatient services to patients of all applicable age groups including infant child adolescent adult and geriatric patients. Acts as educator consultant and liaison to other healthcare providers across the care continuum.


Demonstrates a fundamental grounding in nursing theory and practice with mastery of knowledge within the defined content area.

Responds to/documents telephone and in-office triage according to guidelines as appropriate

Oversees care coordination in ambulatory care settings.

Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics.

Performs advanced practice skills in accordance with office or department assignment.

Assesses patient health status:

Obtains relevant health/medical history

Performs physical examination based on age /history

Conducts screening procedures based on age /history

Identifies medical/health risks and needs

Updates/records changes in health status

Establishes diagnosis

Formulates appropriate differential diagnosis based on history physical examination clinical findings

Identifies needs of individual family community

Develops treatment plan:

Orders/performs/interprets tests and procedures as indicated by diagnosis and age

Identifies appropriate pharmacologic agents

Identifies non-pharmacologic interventions

Collaborates with physician per protocols and/or consultation

Develops patient education plan

Implements plan based on age specific needs

Bases plan on scientific principles theoretical knowledge and clinical expertise

Executes plan safely/effectively

Correlates method with practitioner�s experience

Initiates actions

Records visits in concise clear logical manner

Performs follow-up/evaluation

Coordinates directs implements evaluates patient/family teaching

Exhibits leadership

Identifies impeding problems/formulates intervention

Utilizes times management in performing patient services

Maintains consistent productivity level according to access practice load complexity of problems

Analyzes/adapts to new/changing situations

Collaborates with team members to facilitate smooth office operations

Other information:

Rhode Island Nurse Practitioner License with prescriptive authority.

Graduate of a Baccalaureate and Master�s program in Nursing.

National certification in family or adult medicine in accordance with department assignment.

Current certification in Basic Life Support.

Physical Requirements:

Must be able to stand and walk 3-7 hours per day sit/squat/kneel 1-3 hours per day lift over 10-35 lbs with good body mechanics occasional twisting /bending.

Must be able to use both hands /wrists and perform work that deviates from neutral position and be able to perform frequent fine motor skills.

Must be able to have accurate near and far vision of 20/40 and accurate depth perception and have both red-green and blue-violet discrimination.

Must hear a whisper at 8 feet.
