Nurse Practitioner

Summary:
Responsible for the day-to-day and overall operations of the Wound Treatment Program including responsibility for operations reimbursement and quality management. Collaborates with the Medical and Administrative Directors and other healthcare team members to provide consistent quality care for wound program treatment patients. Provide direct supervision to program staff members. Provide expert resource educational services and direct patient care.

 

Responsibilities:
Program Operations/Human Resources Management:

Collaborates with Medical and Administrative directors to maintain the wound treatment program including the review and revision of patient protocols documentation tools and wound care formulary consistent with current evidence of best practice and national standards.



Regularly reviews and recommends revisions to standards of care competencies policies and procedures in quality cost efficient and effective wound care for all hospital clinical staff.



Coordinates and participates in performance of monitoring and improvement activities related to wound care treatment program on on-going basis.



Develops and maintains wound treatment program policies and procedures.



Communicates with appropriate Hospital personnel to ensure adequate space equipment and supplies.



Assists with development of annual program budget.



Reviews operational data and develops regular reports.



Performs operational responsibilities related to hospital reimbursement.



Maintains program compliance with all applicable regulatory agencies.



Collaborates with Nursing Professional Development Coordinator to develop and provide formal educational experiences for nursing and other hospital staff.



Collaborates with Hospital�s Marketing Department in the development and implementation of the Wound Treatment Program Marketing Plan to encourage patient referrals to the Program



Recruits selects trains and manages the performance of wound treatment program staff:

- Completes annual performance reviews

- Recommends salary changes based on performance

- Documents problems/ recommends or initiates corrective action

- Develops and maintains position descriptions



Provides daily supervision to program staff members.



Conducts regular program staff meetings to ensure open communication and enhance operational efficiencies.



Participates in professional organizations.



Clinical / Patient Care Responsibilities:

Serves as expert resource and specialty consultant to all clinical staff to effectively assess wounds and recommend appropriate protocols.



Monitors inpatient population for potential referral to wound treatment program.



Remains current regarding professional practice standards maintains and develops own skills and applies to patient care.



Maintains knowledge and exercises clinical competencies of comprehensive wound management.



Collaborates with multi-disciplinary teams to assess patient needs for nursing care patient and family education and other services.



Provides case management role for patients of wound treatment program.



Provides assessment of wounds and ostomy care needs.



Documents and integrates data into patient�s medical record.



Provides direct patient care in accordance with Hospital policies and procedures.



Ensures continuity of care through appropriate discharge planning coordination of services and education.



Makes recommendations to physicians for changes to wound care orders as needed.



Educates patients and families regarding wound/ostomy care to facilitate recovery.



Maintains current BLS certification.

Qualifications:

Bachelor�s degree in Nursing. RN with RI RN License. Nationally recognized certification in wound care or expectation to obtain within twelve months of hire. Five years of progressive clinical medical-surgical nursing experience including expertise in wound/ostomy care. Current BLS Certification. Excellent written and oral communication skills. Effective organizational analytical and computer skills. Basic understanding of healthcare reimbursement. Demonstrated leadership and supervisory skills.
