Speech Pathologist PD

Summary:
Under the direction of the Senior Staff Clinical Specialist Clinical Coordinator or Clinical manager of Speech Language Pathology provides clinical service including screening assessment diagnosis intervention management counseling/education documentation and referral for inpatients and outpatients demonstrating speech voice feeding swallowing language and cognitive impairments. Demonstrates competency in caring for individuals across the entire age span which may include the neonate/infant toddler preschool and school-aged children adolescent adult and geriatric populations using patient/caregiver centered focus and applying basic concepts of evidence-based practice in clinical care to individuals from diverse language ethnic cultural and socioeconomic backgrounds as well as to individuals who have multiple disabilities. Provisions of service are in accordance with state laws and regulations and standards of practice as set forth by the American Speech Language Hearing Association (ASHA).

Communication:

Exhibits effective and professional verbal and non-verbal communication skills in accordance with Rhode Island Hospital’s customer service standards. Communicates clearly respectfully and honestly with patients families caregivers and colleagues. Works collaboratively within the academic medical center and more broadly with community-based healthcare providers. Proactively provides written and verbal communication and recommendations to physicians nurses case managers and other personnel involved in the patient’s care.

Adjusts communication and response to the level of understanding of the listener.

Clinical Responsibilities:

Consistently applies the basic concepts of evidence-based practice to all aspects of clinical care. Completes all necessary clinical competencies as assigned by the clinical manager. Seeks additional knowledge when necessary to assure safe and effective treatment of all patients.

Evaluates and diagnoses patients demonstrating speech voice language feeding/swallowing and cognitive impairments based on patient history behavioral observations and results of tests and procedures. Uses assessment data to guide clinical decision making in determining the plan of care. Provides appropriate services to patients consistent with their level of medical acuity. Makes recommendations and appropriate referrals for additional diagnostic testing to ensure appropriate management. Prepares report of findings indicating evaluation results diagnosis and prognosis.

Demonstrates knowledge of medical speech-language pathology practice.

Demonstrates knowledge of patient social and medical factors that contribute to the complexity of patients including but not limited to respiratory cardiac renal metabolic and neurologic systems ethics and end of life issues.

Develops selects and prescribes multimodal augmentative and alternative communication systems including unaided strategies (e.g. manual signs/gestures) and aided strategies (e.g. speech-generating devices manual communication boards picture schedules). Trains and educates individuals families and other communication partners in use.

Selects fits and establishes effective use of appropriate prosthetic/adaptive devices for communication and swallowing (e.g. electrolarynges speaking valves.).

Uses instrumental technology to diagnose and treat disorders of communication and swallowing (e.g. Videofluoroscopy for speech and swallowing Fiberoptic Endoscopic Evaluation of Swallowing Videostroboscopy Videnonasophagyngoscopy neuromuscular electrical stimulation computer technology).

Provides services to individuals with hearing loss and their families/caregivers (e.g. auditory training for patients with cochlear implants and hearing aids; speech and language intervention secondary to hearing loss).

Provides assessment and intervention for individuals diagnosed with auditory processing disorders.

Screens individuals for hearing loss or middle ear pathology using conventional pure-tone air conduction methods.

Provides services to modify or enhance communication performance (e.g. accent modification care and improvement of the professional voice).

Develops plan of care designed to meet individual needs of patient. Arranges patient’s therapy schedule. Conducts therapy individually or in group situations according to prepared plans.

Regularly assesses patient’s progress and revises plan of care accordingly in order to meet needs of individual patients. Completes accurate and timely clinical documentation and charges in full compliance with hospital professional and regulatory standards.

Measures outcomes of treatment and conducts continuous evaluation of the effectiveness of practices and programs to improve and maintain quality of services. Maintains documentation for Performance Improvement practices and programs.

Provides speech pathology consultation to physicians rehabilitation personnel and other professionals to direct and coordinate patient’s plan of care.

Provides training/instruction to appropriate members of the department related to clinical area of focus/interest. Provides teaching/in-service/lectures to other hospital departments/personnel students of all levels and outside agencies/schools.

Obtains continuing education units (CEU’s) to maintain Rhode Island Licensure and ASHA certification.

Other information:
QUALIFICATIONS:

Master's Degree in Speech Pathology from an accredited university. State License issued by the Rhode Island Department of Health or Provisional Rhode Island State License for Clinical Fellows involved in supervised clinical fellowship year.

Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA). For clinical fellows evidence of pursuit of ASHA certification.

One to two years’ experience or eligibility for provisional license from the State of Rhode Island. Clinical Fellows will be considered.

Physical Requirements:

Fit for duty as documented by OSHA standards and program specific needs.
