Team Leader Outreach

Summary:
To provide supervision and coordination of the Health Home Team which is designed to enhance the quality of life for dually diagnosed (psychiatrically disabled/chemically dependent) adults. Conduct substance abuse treatment with clients in need according to the 12 core functions of substance abuse counseling as defined by the ICRC.

 

Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:

Meets regularly with assigned Health Home staff to monitor productivity documentation and quality and thoroughness of care given to clients.



Addresses problems in Health Home staff performance by discussion development of strategy to improve and monitoring of employee's efforts to improve. Takes disciplinary action as needed.



Educate Health Home staff concerning issues of substance abuse.



Provides instruction and encouragement as necessary to facilitate optimal staff performance.



Completes performance evaluations for Health Home staff as required by agency.



Train staff with regard to all Center and department's activities and needs in order for the department to function in a productive comprehensive and cost effective manner without compromising quality.



Provides supervision to Health Home staff through regularly scheduled meetings.



Assists psychiatrists in admission decisions through interview and record review.



Coordinates staff schedules to follow standards and to provide best coverage for consumers.



Facilitates daily meetings to review daily log consumer schedules and make daily assignments.



Staff training/practice needs as they relate to substance abuse issues.



Assists New staff with orientation to Health Home services.



Completes Health Home staff evaluations as required by agency policy.



Participates the hiring and terminating of staff for the Health Home program in coordination with Program Manager VP and HR.



Maintain caseload of clients necessary to meet and/or sustain substance abuse credentials.



Coordinate activities make appointments and may include accompanying client(s) as noted on Treatment Plan.



*Services: medical psychiatric substance abuse psychotherapy housing entitlement income maintenance social leisure educational and employment.



Objectives of services to ensure clients active participation in treatment maintenance of a stable residence in community development of ADL skills and development of social support network and pursuit/obtainment of employment.



Coordinate treatment and groups for clients.



Maintain appropriate MIS reports and schedules.



Treatment plan written in accordance with policies and department procedure manual.



Treatment plans to provide optimal functioning in community and health of client.



Appropriate level of service per individual needs which may include accompanying client coordinating activities making appointments.



Document clients� progress notes in prescribed format and according to policies and department procedure manual..



Document all communication with treatment facilities.



Document signed and approved release forms and client rights must be in clients' records.



When providing therapeutic/counseling/case management services arrives and/or participates on time as scheduled; is alert and responsive to client issues and behaviors.



Participate in daily staff organizational meetings and treatment planning review meetings.



Provide ongoing assessment of client�s mental illness symptoms and client�s response to treatment.



Provide ongoing assessment problem solving side-by-side services skill training supervision (i.e. assignments monitoring encouragement) and environmental adaptations to assist clients with activities of daily living.



Attends of agency CSS Health Home Staff Meetings as scheduled.



Participates in direct contact with clients family and collateral contacts according to treatment goals. A minimum of eight hours per week of direct service with clients.



Weekly supervisory sessions as scheduled.



Prepares for supervision by completing assigned tasks formulation questions and field supervision.



Attends agency or supervisory required inservices.



Attends inservices as indicated by requirements of agency.



Attends at least one course seminar or conference per year for improvement of job skills.



Participates in at least one agency committee subject to needs of agency.



Participates in administrative backup responsibility for after hours calls by carrying pager on a rotating basis and consulting with ESS staff.



Other duties and responsibilities as assigned or needed.



In addition to listed primary Team Leader responsibilities the Team Leader will provide assistance with and facilitation of psychiatric rehabilitation related assessment(s) interventions and services that will facilitate the client�s achievement of their Recovery and Overall Rehabilitation Goals as required by and indicated on the Client�s Master Treatment Plan. When assessing planning for and providing Health Home services the individual along with assistance and consultation from the Health Home Rehabilitation Specialist and with supervision from team leadership must maintain a strong focus and prioritize Rehabilitative related service activities in order to maximize the client�s functional outcome(s) achievement with chosen roles within chosen environments and overall potential for maximum recovery. Some of the additional duties and responsibilities required to fulfill this role are but not limited to:



Utilize Stages of Change theory Motivational Enhancement techniques and Rehabilitation Goal Readiness & Development Technology & Process to facilitate maximum potential for rehabilitation goal Attainment.



Become familiar with and provide consultation on client subsidy work incentives and program such as the impact of work on SSDI SSI Medicaid and Hosing Subsidies.

Provide assistance to clients as needed with the completion of documentation administrative tasks and processes related to the client obtaining employment accessing educational programs and other community organization(s) related to their rehabilitation goal(s) such as completing Job Applications(s) financial aid packages etc.

Assist client(s) and rehabilitative specialist(s) with required job educational and social/leisure development services and activities as needed.



Assist team Rehabilitative Specialist with providing on/off site supports assessed as needed on order to facilitate client job and/or client meaningful activity retention satisfaction and success.



Provide interventions and services designed to assist the client with obtaining satisfying social leisure opportunities and community activities that maximize the cline�s level of community integration as well as assist the cline in obtaining and maintaining a recovery promoting natural support system and peer relationships.



Other duties as assigned.

 

Other information:
QUALIFICATION REQUIREMENTS:

Master's Degree in related field (MSW preferred) with at least two years of administrative and clinical experience with dual-diagnosed (psychiatrically disabled/chemically dependent) adults;

or Registered Nurse with Diploma from recognized School of Nursing or Bachelor's Degree in Nursing and at least two years of psychiatric nursing experience or one year of psychiatric nursing experience and course work toward a graduate degree.



NECESSARY SPECIAL REQUIREMENTS:

Certified or certifiable as a chemical dependency professional in the State of Rhode Island.

Valid driver's license and use of car during normal working hours or other means of transportation to complete major functions of job.

Automobile liability insurance coverage for vehicle being used in accordance with state regulations.

CPR training within 90 working days of employment and maintained annually thereafter.

Master's Degree

Rhode Island Certification as a Mental Health Case Manager within one year of date of hire (if indicated).



JOB KNOWLEDGE SKILL AND ABILITY:

Ability to work days evenings weekends and holidays as assigned.

Understanding of chemical dependency treatment issues according to the 12 core functions of counseling as defined by the ICRC.

Understanding of mental health issues.

Knowledge of community resources.

Knowledge of State Entitlement Program.

Ability to successfully complete all required training and certifications

Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.

Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals either manually or by use of a calculator.
