Medical Records Clerk

Summary:
The Medical Records Clerk reports to the Health Information Services (HIS) Manager. Receives assembles and analyzes inpatient and outpatient medical records. Performs a variety of clerical analytical data entry retrieval and filing duties in support of medical record processing functions. Maintains patient confidentiality at all times.

 

Responsibilities:
Retrieves discharged medical records from various nursing units and ancillary departments. Assists in reconciling retrieved medical records against patient discharge listing to ensure receipt of all records within 24 hours of discharge /visit.



Assembles medical records according to established chart order and reviews chart for any missing reports or paperwork; follows up with appropriate department. Notes and corrects any difference in medical record number or color code on folder. Identifies duplicate medical record numbers and reports them appropriately to the Enterprise Access Directory (EAD) Coordinator for correction.



Scans define set of documents into document imaging system. Performs quality review of documents scanned by other departments.



Reviews inpatient and outpatient medical records to ensure that all physician documentation is completed in compliance with the Medical Staff Rules Regulations and Bylaws The Joint Commission (TJC) standards and federal and state guidelines. Retrieves reports needed for a complete quantitative review for various patient types.



Assigns incomplete medical record deficiency to each report for physician signature; enters deficiency into department Chart Fact system.



Assists Health Information Services personnel in various duties to avoid backlogs.



May be required to provide front desk/receptionist coverage.



Reports weekly workload statistics to Department Leads.



Performs other related duties as assigned.

 

Other information:
BASIC KNOWLEDGE:

High School Diploma or equivalent required.



Proficiency in Microsoft Office applications including Access Excel and Word.



EXPERIENCE:

Two years in an office environment with electronic medical records experience preferred.



Knowledge of medical terminology preferred.



Customer Service skills required.



Must be detail-oriented.



WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS:

Typical office environment with no exposure to adverse environmental conditions.
