Physical Therapist PD

Summary:
May provide physical therapy services to patients of all ages including children adolescents adults and geriatric patients. Evaluates the physical therapy needs of the patient develops goals and executes treatment plan. Provides education to patients families and caregivers. Supervises physical therapy students physical therapist assistants and support staff in compliance with hospital and state regulatory policies.

EVALUATION SKILLS

Evaluates rehabilitation patients utilizing appropriate tests and measurements for age and condition.

Demonstrates ability to assess age-specific or special needs of rehabilitation patients:

- Neonates/Infants up to age 1

- Children (1-11)

- Adolescents (12-17)

- Adults (18-55)

- Geriatric (55+)

Develops goals and treatment plan for rehabilitation patients following physician referral hospital and departmental policies and procedures professional standards of practice and CMS compliance guidelines.

TREATMENT SKILLS

Re-evaluates and updates goals/plan in accordance with patient needs or at least every 30 days per hospital and regulatory policies.

Implements appropriate treatment techniques for rehabilitation patients of any diagnosis.

Addresses age specific treatment needs of rehabilitation patients:

- Neonates/Infants up to age 1

- Children (1-11)

- Adolescents (12-17)

- Adults (18-55)

- Geriatric (55+)

Develops and implements patient/family education plan.

Coordinates plan of care with all members of the healthcare team patient family and caregiver as appropriate.

Communicates patient�s status/progress to all members of the treatment team.

Follows department processes for communicating patient�s scheduling needs. Prepares clear written patient coverage information proceeding planned time off.

Terminates patient treatment based on patient condition/progress and within appropriate time frames. Utilizes ABN if applicable to setting.

Demonstrates proper operation of position-specific equipment and/or procedures.

DOCUMENTATION SKILLS

Documents in the medical record accurately concisely legibly and in a timely manner both in writing and on the computer as required.

Consistently complies and is timely with processes related to authorization reauthorization and financial coverage of physical therapy services.

Complies with department timekeeping practices.

Charges appropriately for services (including modifiers) and supplies.

Supervises Physical Therapist Assistant in accordance with hospital and state regulatory policies.

Presents inservice after attending all hospital-sponsored courses.

LEVEL II - STAFF

Evaluates and treats patients with a wide variety of diagnoses with minimal supervision.

Demonstrates ability to cover any service within individual�s control hours including weekends and holidays.

Demonstrates active participation in continuing education to maintain or add to skill level through the development of an educational plan that might include use of a mentor in-services seminars journal article review conference attendance etc.

Participates in committees or project teams to benefit hospital and/or department as recommended by director/supervisor.

LEVEL III - RESOURCE CLINICIAN

Evaluates and treats patients with a wide variety of diagnoses independently.

Provides formal teaching to department staff individually or in groups.

- Specifies learning objectives and designs educational program to meet the objectives.

Demonstrates a broad involvement in specialty area by promoting the mission of the hospital through involvement in professional and/or community activities.

LEVEL IV - ADVANCED CLINICAL RESOURCE

Demonstrates the ability to develop and lead a clinical specialty including development of special programs.

Demonstrates involvement in chart review quality assurance and performance improvement activities.

Evaluates and treats a sufficient number of cases to maintain proficiency.

Provides clinical supervision when requested.

Actively participates in national state or local professional organizations study groups or committees.

Demonstrates leadership skills for staff development including the development and assessment of staff competencies.

 

Other information:
Qualifications:

Bachelor�s Master�s or Doctorate degree in physical therapy or a certificate of physical therapy from an approved college or university.

License to practice physical therapy in Rhode Island

Current CPR certification

Must be able to sit stand squat/kneel for 1-3 hours per day walk 3-7 hours per day lift over 50lbs with good mechanics twist/bend frequently. 

Must be able to use both hands/wrists and perform work that deviates from neutral position and be able to perform fine motor skills. 

Must have accurate near and far vision 20/40 depth perception and discriminate among colors. 

Must be able to hear whispers at 8 feet.
