Registered Nurse

Summary:
To provide medical and clinical consultations to dually diagnosed (psychiatrically disabled/ chemically dependent) clients in the IHH. Conduct substance abuse treatment with clients in need according to the 12 core functions of substance abuse counseling as defined by the ICRC.

 

Responsibilities:
MAIN DUTIES AND RESPONSIBILITIES:

Assess Develop and Implement treatment plans written in accordance with policies and department procedure manual.



Focus is to promote optimal functioning and health of client.



Ensure appropriate level of care per individual by accompanying client making appointments providing supportive counseling case management and coordinating medical referrals as needed.



Document service provision via progress notes written in accordance with policy and department procedure manual.



Medical record documentation must adhere to Gateway policies and procedures and IHH Medicaid guidelines.



Provide counseling referral to appropriate services.



Address medical/psychiatric emergencies of clients and staff.



Provide appropriate assessment in addressing crisis as per procedure.



Document according to standard procedure.



When providing therapeutic/counseling/case management services arrives and/or participates on time as scheduled; is alert and responsive to client issues and behaviors.



Provide ongoing assessment of client�s mental illness an medical symptoms and client�s response to treatment.



Minimum of 22 hours/week in direct contact with client.



Records will be updated in accordance to department procedure.



Treatment plans will be updated in accordance with department procedure.



Monitor psychosocial functioning and ongoing need for pharmacological intervention.



Provide crisis intervention brief counseling or case management when determined necessary.



Consult/provide medical/psychiatric consultation to department(s) as appropriate to position.



Consult/provide individual and/or group substance abuse treatment.



Consult/provide Case management activities as required to address caseload needs.



Administering of meds when ordered by Agency physician.



Will evaluate clients in crisis for ability to benefit/respond to emergency medication.



Monitor medication to ensure client understands prescribed dosage



Dispense medication according to physician orders and in accordance with policy and procedure manual.



Document medication according to Gateway policies and procedures.



Adhere to medication room procedures manual (as developed by pharmacist).



Maintain inventory of medication/medical supplies.



Monitor inventory and order as needed.



Maintain inventory in accordance with policies and procedures.



Monitor first aid kits once a month.



Attends agency or supervisor required inservices.



Attends other agency inservices.



Attends at least one course seminar or conference per year for improvement of job skills.



Attends Team Staff Meetings as scheduled.



Attends supervisory sessions as scheduled.



Prepares for supervision by completing tasks assigned and formulating questions.



Other duties and responsibilities as assigned or needed.

QUALIFICATION REQUIREMENTS:

Registered Nurse with Diploma from recognized School of Nursing Associates Degree or Bachelor's Degree in Nursing and at least two years of psychiatric nursing experience; or one year of psychiatric nursing experience and course work toward a graduate degree.



Certification as a Chemical Dependency Professional or Counselor in Training (CIT) (or working toward certification). Hold a valid and current Rhode Island Registered Nurse License.



NECESSARY SPECIAL REQUIREMENTS:

Valid drivers license and use of car during normal working hours or other means of transportation to complete major functions of job.



Automobile liability insurance coverage for vehicle being used in accordance with Center requirements.



CPR training within 90 working days of employment and maintained annually.



Attain QMHP certification.



JOB KNOWLEDGE SKILL AND ABILITY:

Ability to work days evenings weekends and holidays as assigned.



Working knowledge of chemical dependency and differential diagnosis.



Knowledge of Community Resources.



Ability to successfully complete all required training and certifications



Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication.



Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals either manually or by use of a calculator.



Attain Qualified Mental Health Professional certification to facilitate IHH client involuntary hospitalizations when indicated.
