Baccalaureate degree in nursing, or baccalaureate degree in health services administration, public health or related field with relevant clinical experience in hospital environment required. Advanced degree in related field required. Certified RCA facilitator preferred. Strong verbal/written and presentation skills required. Six Sigma/Lean Green Belt or Black Belt certification desired, or completed within 18 months of accepting position.


Three years of relevant supervisory/management experience in nursing, or patient safety programming, risk management, conducting FMEAs and use of root cause analysis tools, facilitation of small and large groups/project teams, performance improvement systems and/or process improvement methods and standards. Project management experience, preferably in a large complex healthcare organization is desired. Knowledge of the SafetyNet event reporting system and The Joint Commission patient safety standards is required.

The Manager of Patient Safety reports to the Lifespan Director of Patient Safety. Under general supervision and within Lifespan established policies and procedures, manages coordination, collection and analysis of patient safety data for internal comparison and external reporting through the electronic event reporting system (e.g., SafetyNet). Coordinates system efforts and provides support for the event reporting workflow to ensure appropriate, timely identification of events requiring root cause identification and analyses, and related improvement project timelines. Supervises the Patient Safety Specialists and is responsible for assisting the department director with coordinating and ensuring smooth operational processes. Establishes and maintains systems to ensure timely event review to ensure relevant event data submission to the patient safety organization via the event reporting system. Monitors for and facilitates implementation of changes to ensure effective quality assessment/ performance improvement (QAPI) monitoring requirements of the hospital safety programs, The Joint Commission (TJC), Centers for Medicare and Medicaid Services (CMS), RI Department of Health and other regulatory agency requirements are met. Collaborates with Risk Management and supports tracking and reporting systems ensuring the effectiveness of corrective actions identified through root cause analysis and improvement opportunities.
