High school graduate or equivalent. Training in specific research project education and bilingual strongly preferred. Demonstrated knowledge and skills necessary to provide care to clients throughout the life span with consideration of aging processes, human development stages and cultural patterns in each step of the care process.


Two years’ of social services or other related experience. Data entry skills and PC experience required.
Demonstrated knowledge and skills necessary to work with highly marginalized patient populations. Training knowledge and experience with HCV, and injection drug use/addiction-related issues. Professional and analytical skills in evaluating patients in order to plan and provide patient care and monitor treatment plan for each patient.

Under the general supervision of the Principal Investigator and/or Project Director, performs a wide variety of administrative and counseling duties to provide patient intake, outreach, discharge planning, and follow-up. Acts as a liaison to internal hospital departments as well as state and federal agencies that provide social services and community services. All essential functions performed in this position reflect the age specific needs of the patients.
